Hi

We are creating a Texas Home Inspection Form.  In Texas it is required that you use a specific form which can be recreated in word, but it has to remain in the same format..   On this form there are some check boxes in each category that we have to check if a specific item in the house was:

• Inspected (I)
• Not Inspected (NI)
• Not Present (NP)
• Deficient (D)

I am attaching a screenshot of the form as an example.

I am a beginner with The Form Tool and am finding it pretty easy to use but I just cant seem to wrap my head around how to create a condition to check the check boxes if the answers to the question is yes.  It has to remain in the format shown in the Attachment.  I have tried to set up a condition to insert a symbol that looks like a check box but that just doesn't seem to work.

Any help will be appreciated,
