TheFormTool in a Text Box?

TheFormTool fields cannot be inserted within textboxes (or within shapes either). They do work within headers, footers, footnotes, and endnotes; but not within textboxes or shapes. One pretty good work-around is if you're able to "simulate" a textbox by creating a 1x1 table and positioning it in the desired location on the page.
